We are seeking a highly organized, compassionate, and proactive Registered Nurse (RN) to coordinate care within our 16-bed skilled nursing facility (SNF) rehab unit. The Rehab Care Coordinator serves as a key liaison between residents, families, healthcare providers, and referral sources. This position is responsible for overseeing the resident journey from admission through discharge, ensuring a smooth, supportive, and high-quality rehab experience.

This role is ideal for a nurse who thrives in a fast-paced environment, values strong relationships with patients and families, and excels in communication and follow-through.

Key Job Responsibilities Include:

· Conduct resident rounds twice weekly to ensure care needs are met

· Conduct comprehensive discharge planning

· Provide ongoing support, education, and communication to residents and their families

· Maintain a consistent presence with local hospitals and referral partners to support admissions and relationships

This position is full-time and will work 40 hours per week. This position will also be required to be on call one week per month. Must also be willing to work as a floor nurse as needed.

Qualifications/Requirements:

· Registered Nurse (RN) license in good standing

· Experience in skilled nursing, rehab, or care coordination preferred

· Strong organizational and multitasking abilities

· Excellent interpersonal, communication, and problem-solving skills
